You are intubating a patient using a gum elastic bougie. While trying to pass the endotracheal tube over the bougie, it becomes held up. What do you do?

Because of its shape, your tube may be held up on the arytenoid cartilages. The video above demonstrates why this happens. Your best option at this point is to attempt a “flip-flop maneuver”. To do this, back the tube up, turn it 90 degrees counter-clockwise, and advance again. This will put the bevel of the tube down and may allow it to pass over the arytenoids.
