Farmakovijilans tedbirleri ve bildirimleri kapsam\u0131nda pandemi s\u0131ras\u0131nda kulland\u0131\u011f\u0131m\u0131z ila\u00e7lar\u0131n ne kadar yan etkilerinin ve toksik etkilerinin bildirildi\u011fini sizler de merak etmi\u015fsinizdir. <strong><a href=\"https:\/\/www.ahajournals.org\/journal\/circ\"><em>Circulation <\/em><\/a><\/strong>dergisinde 10 Haziran 2020\u2019 de yay\u0131nlanm\u0131\u015f hen\u00fcz taslak halinde olan bir derleme mevcut.<\/p>\n<p><strong>Lee S. Nguyen<\/strong> ve arkada\u015flar\u0131 bu yaz\u0131y\u0131 <em>(<a href=\"https:\/\/www.ahajournals.org\/doi\/abs\/10.1161\/CIRCULATIONAHA.120.048238\">Cardiovascular Toxicities Associated with Hydroxychloroquine and Azithromycin: An Analysis of the World Health Organization\u00a0Pharmacovigilance Database<\/a>)<\/em> Covid- 19 d\u00f6neminde kullan\u0131lan ila\u00e7lar\u0131n y\u0131llard\u0131r bildirilmi\u015f advers etkilerini Farmakovijilans veritabanlar\u0131n\u0131 tarayarak haz\u0131rlam\u0131\u015flar. Yazarlar\u0131n amac\u0131; \u00f6zellikle Covid-19 pn\u00f6monisinde kullan\u0131lan <strong>Hidroksiklorokin<\/strong> ve <strong>Azitromisin<\/strong> ila\u00e7lar\u0131n\u0131n birlikte veya tek ba\u015f\u0131na kullan\u0131mlar\u0131n\u0131n ne s\u0131kl\u0131kta kardiyotoksik etkilere sebep oldu\u011funu saptamak. Bu retrospektif g\u00f6zlemsel \u00e7al\u0131\u015fma i\u00e7in kullan\u0131lan VigiBase\u00ae; WHO taraf\u0131ndan kullan\u0131lan bir veritaban\u0131d\u0131r ve 130 \u00fclkeden 21 milyon veriyi kapsamaktad\u0131r. Bu iki ilac\u0131n tek veya kombine kullan\u0131m\u0131 i\u00e7in tarama yapm\u0131\u015flar (clinicaltrials.gov identifier NCT04314817). Veriler Kas\u0131m 1967 ve 1 Mart 2020 aras\u0131ndaki zaman d\u00f6nemini kaps\u0131yor.<\/p>\n<p style=\"text-align: center\"><img decoding=\"async\" style=\"height: 400px;width: 800px\" src=\"https:\/\/tatd.org.tr\/tatdtoks\/wp-content\/uploads\/sites\/36\/2021\/10\/vigibase-1.png\" alt=\"\" \/><\/p>\n<p style=\"text-align: center\"><u><strong>Fig\u00fcr 1:<\/strong><\/u> VigiBase\u00ae\u00a0ak\u0131\u015f \u015femas\u0131<\/p>\n<p>Veritabanlar\u0131ndan \u00e7ektikleri 76,822 istenmeyen advers etkide Hidroksiklorokin \u00a0monoterapisi,\u00a0\u00a0 89,692 advers\u00a0 etkide Azitromisin monoterapisi \u00a0ve 607 advers\u00a0 etki vakas\u0131nda ise kombine tedavi kullan\u0131ld\u0131\u011f\u0131n\u0131 saptam\u0131\u015flar.\u00a0 Taranan total advers etki say\u0131s\u0131 ise 21,275,867 VigiBase\u00ae bildirisi olarak dikkat \u00e7ekmekte. Birden \u00e7ok ila\u00e7 kullan\u0131lan vakalarda Hidroksiklorokin \u015f\u00fcphe edilen madde olarak 21,808\/76,822 (%28.4) ve Azitromisin 54,533\/ 89,692 (%60.8) vakada bildirilmi\u015f.<\/p>\n<p style=\"text-align: center\"><img decoding=\"async\" style=\"height: 400px;width: 800px\" src=\"https:\/\/tatd.org.tr\/tatdtoks\/wp-content\/uploads\/sites\/36\/2021\/10\/122-1.png\" alt=\"\" \/><\/p>\n<p style=\"text-align: center\"><u><strong>Fig\u00fcr 2:<\/strong><\/u> Y\u0131llara g\u00f6re\u00a0Hidroksiklorokin ve\u00a0Azitromisin i\u00e7in ila\u00e7 yan etki (ADR) bildirimleri\u00a0(<a href=\"http:\/\/www.ahajournals.org\/doi\/abs\/10.1161\/CIRCULATIONAHA.120.048238\">1<\/a>)<\/p>\n<p>Yaz\u0131 \u00e7ok k\u0131sa ve t\u00fcm verileri eksik haz\u0131rlanm\u0131\u015f. Vakalarda ne gibi advers etkiler oldu\u011fu net de\u011fil. Azitromisin alan 480 hastada ve Hidroksiklorokin alan 136 hastada QT uzamas\u0131 ve\/veya Torsades-de-Pointes (TdP\/VT) gibi ventrik\u00fcler ta\u015fikardiler tespit edilmi\u015f. Azitromisin alan 223 hastada\u00a0 uzam\u0131\u015f QT 223\u00a0 ve\u00a0 257 vakada TdP\/VT ise 257 hastada tespit edilmi\u015f. Hidroksiklorokin alan 136 hastan\u0131n\u00a0 53\u2019 \u00fcnde uzam\u0131\u015f QT ve 83\u2019 \u00fcnde \u00a0TdP\/VT tespit edilmi\u015f. Hidroksiklorokin alan hastalarda AV blok veya dal bloklar\u0131 (n=75, IC025=1.04) ve kalp yetmezli\u011fi de tespit etmi\u015fler (n=203, IC025=0.06). Azitromisine ba\u011fl\u0131 geli\u015fen uzam\u0131\u015f QT2li vaka say\u0131s\u0131 736\/ 89,085 (%0.8) iken, Hidroksiklorokinde bu say\u0131 263\/ 76,215 (0.3%) (ROR=2.36, %95 CI=2.05-2.71) olarak g\u00f6zlenmi\u015f. Kombine tedavide oran daha da artmakta. \u00a0Monoterapide 999\/165,300 (%0.6) olan oran\u0131n, kombine terapide 9\/ 607 (%1.5) d\u00fczeyinlerine kadar y\u00fckseldi\u011fi ortaya \u00e7\u0131km\u0131\u015f (ROR=2.48, %95 CI=1.28- 4.79).<\/p>\n<p>\u00d6l\u00fcm oranlar\u0131na gelirsek; Hidroksiklorokin alanlar i\u00e7inde TdP\/VT vakalar\u0131nda mortalite %8.4 (7\/83) iken, Azitromisin alanlarda mortalite %20.2 olarak not edilmi\u015f (p&lt;0.001).
