Bu hastalar\u0131n hayatta kalabilmesi esas olarak \u00e7\u0131\u011fda g\u00f6m\u00fclme s\u00fcrelerine ba\u011fl\u0131d\u0131r. \u00c7\u0131\u011f alt\u0131nda kalan hastalar\u0131 3 fazda inceleyebiliriz. Ana \u00f6l\u00fcmler <strong>travmatik yaralanmaya<\/strong> ba\u011fl\u0131 ilk fazda olu\u015fur. \u0130kinci fazda <strong>asfiksi<\/strong>, \u00fc\u00e7\u00fcnc\u00fc fazda ise <strong>hipotermi<\/strong>, <strong>hipoksi<\/strong> ve <strong>hiperkapni <\/strong>geli\u015ferek \u00f6l\u00fcme yol a\u00e7abilir. Tabi bunlar\u0131 d\u00f6rd\u00fcnc\u00fc bir faz olarak kabul edilebilen <strong>uzun s\u00fcreli hayatta kalma<\/strong> a\u015famas\u0131 takip edebilir. \u0130lk 60 dakika tam g\u00f6m\u00fclmeden sonra hayatta kalma \u015fans\u0131 dramatik bir \u015fekilde %21\u2019 d\u00fc\u015ferken, 60 ile 180 dakika aras\u0131nda 4. fazda ise d\u00fc\u015fen e\u011fri plato \u00e7izebilir. Kardiyak arrest ge\u00e7iren \u00e7\u0131\u011f kurbanlar\u0131n\u0131n hayatta kalma olas\u0131l\u0131\u011f\u0131 d\u00fc\u015f\u00fck olsa da, uzun s\u00fcre g\u00f6m\u00fcl\u00fc kalan ve \u015fiddetli hipotermisi olan kurbanlarda tam res\u00fcsitasyon ve ekstra korporeal ya\u015fam deste\u011fi (ECLS) ile yeniden \u0131s\u0131nmas\u0131ndan sonra iyile\u015fme g\u00f6zlenmi\u015f.<\/p>\n<p><strong>Avrupa Res\u00fcsitasyon Konseyi 2021 K\u0131lavuzu \u00e7\u0131\u011f alt\u0131ndaki hasta res\u00fcsitasyonu i\u00e7in \u015funlar\u0131 \u00f6nermektedir:<\/strong><\/p>\n<p><strong>1-<\/strong> En \u00e7ok hipoksi nedenli kardiyak arrest oldu\u011fu i\u00e7in kardiyopulmoner res\u00fcsitasyona 5 ventilasyon ile ba\u015fla,<\/p>\n<p><strong>2-<\/strong> \u00c7\u0131\u011f alt\u0131nda kalma s\u00fcresi &lt;60 dakika ise standart ileri ya\u015fam deste\u011fi uygulay\u0131n,<\/p>\n<p><strong>3-<\/strong> \u00c7\u0131\u011fa g\u00f6m\u00fclme s\u00fcresi &gt;60 dakika olan \u00e7\u0131\u011f kurbanlar\u0131 i\u00e7in, hava yolunun t\u0131kanmas\u0131na veya ilave kurtar\u0131lamaz yaralanmalara dair kan\u0131t olmaks\u0131z\u0131n, ECLS\u2019yi yeniden \u0131s\u0131tma da d\u00e2hil olmak \u00fczere tam res\u00fcsitatif \u00f6nlemler sa\u011flay\u0131n.<\/p>\n<p><strong>4-<\/strong> \u00c7\u0131\u011fa g\u00f6m\u00fclme s\u00fcresi &gt;60 dakika olan ve hava yolunun t\u0131kal\u0131 oldu\u011funa dair ek kan\u0131tlar\u0131 olan kardiyak arrest hastaya suni teneff\u00fcs gereksizdir.<\/p>\n<p><strong>5-<\/strong> Hastanedeki ba\u015far\u0131l\u0131 yeniden \u0131s\u0131tman\u0131n prognozu HOPE (Hypotermia Outcome Prediction after ECLS) skoruna dayanmal\u0131d\u0131r. Geleneksel serum potasyum d\u00fczeyi ve santral ate\u015f e\u015fik de\u011ferleri( s\u0131ras\u0131yla 7 mmol\/L ve 30 \u00b0C) daha az g\u00fcvenilirdir.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>\u00c7\u0131\u011f Olu\u015fmadan \u00d6nce Al\u0131nabilecek Tedbirler<\/strong><\/p>\n<p>\u00c7\u0131\u011fa yatk\u0131n b\u00f6lgelerde veya ge\u00e7mi\u015finde \u00e7\u0131\u011f afeti bulunan b\u00f6lgelere yap\u0131lacak seyahatlerde y\u00fckseklik, e\u011fim, bak\u0131, yama\u00e7 \u015fekli ve arazi kullan\u0131m\u0131 (bitki \u00f6rt\u00fcs\u00fc) gibi parametreler kullan\u0131larak \u00e7\u0131\u011fa yatk\u0131n b\u00f6lgelerin \u00e7\u0131\u011f duyarl\u0131l\u0131k haritas\u0131n\u0131 \u00e7\u0131karmak hayat kurtar\u0131c\u0131 olabilir.<\/p>\n<p><strong>E\u011fer \u00c7\u0131\u011fa Yakalan\u0131rsan\u0131z?<\/strong><\/p>\n<p>\u00c7\u0131\u011f genellikle \u00e7ok h\u0131zl\u0131 geli\u015fir ve durma b\u00f6lgesine gelene kadar hareket eder.
