Tabi bu zor \u015fartlarda i\u015fimizi yaparken bir yandan da akademik bulu\u015fmalar\u0131m\u0131z\u0131 s\u00fcrd\u00fcrmeye \u00e7al\u0131\u015f\u0131yoruz. Bu akademik bulu\u015fmalar\u0131m\u0131z\u0131n olmazsa olmaz\u0131 da yo\u011fun akademik \u00f6zveri yan\u0131nda ila\u00e7 sekt\u00f6r\u00fcn\u00fcn sa\u011flad\u0131\u011f\u0131 ko\u015fulsuz destekler.&nbsp;<\/p>\n\n\n\n<p>4 Eyl\u00fcl 2018 tarihinde T\u00fcrkiye Acil T\u0131p Derne\u011fi (TATD) y\u00f6netim kurulu taraf\u0131ndan d\u00fczenlenen \u201cAcil T\u0131p ve \u0130la\u00e7 Sekt\u00f6r\u00fc \u0130leti\u015fim Bulu\u015fmas\u0131\u201d toplant\u0131s\u0131n\u0131n \u00fczerinden 3 y\u0131ldan fazla zaman ge\u00e7ti. Bu toplant\u0131 ila\u00e7 sekt\u00f6r\u00fcn\u00fcn ve acil t\u0131p camias\u0131n\u0131n daha sa\u011flam temellerde yan yana gelmesi i\u00e7in \u00f6nemli bir ad\u0131m olarak de\u011ferlendirilebilir. Bu \u00f6nemli toplant\u0131da payla\u015f\u0131lan acil t\u0131p prati\u011fi ile ilgili bilgiler bug\u00fcn neredeyse t\u00fcm ila\u00e7 firmalar\u0131nda, en \u00fcst y\u00f6neticiden m\u00fcmessillerine kadar h\u00e2kim olan veriler olarak g\u00f6r\u00fcnmektedir.&nbsp;<\/p>\n\n\n\n<p class=\"rounded-3 px-3 py-2 has-pale-cyan-blue-background-color has-background\"><strong>\u201cArt\u0131k ila\u00e7 firmalar\u0131 acilleri daha iyi tan\u0131yorlar diyebiliriz\u201d<\/strong><\/p>\n\n\n\n<p><strong><em>Peki istenen ileti\u015fim hedeflerine ula\u015ft\u0131\u011f\u0131m\u0131z kabul edilebilir mi?<\/em><\/strong> Bu sorunun yan\u0131t\u0131na bug\u00fcn i\u00e7in hay\u0131r diyebiliriz. Ger\u00e7ekten g\u00fczel geli\u015fmeler de g\u00f6r\u00fclmekte, \u00f6rne\u011fin Genveon ila\u00e7 firmas\u0131 taraf\u0131ndan ilk ulusal acile \u00f6zel tan\u0131t\u0131m ekibi kurulmu\u015f ve t\u00fcm \u00fclke genelinde \u00e7al\u0131\u015fmaya ba\u015flam\u0131\u015ft\u0131r. Abdi \u0130brahim ila\u00e7 deste\u011fi ile TATD taraf\u0131ndan ilk defa \u00e7ok merkezli bir akademik \u00e7al\u0131\u015fma y\u00fcr\u00fct\u00fclm\u00fc\u015f ve yay\u0131n haline getirilmi\u015ftir. Acil servisi do\u011frudan hedef alan kongre ve sempozyum harici kaliteli projeler (Pharmactive &#8211; aktif destek hareketi, Abdi \u0130brahim-uzaktan USG e\u011fitimi, Genveon-Oryantasyon e\u011fitimleri gibi) hayata ge\u00e7irilmi\u015ftir. Ancak bu projeler yine de s\u0131n\u0131rl\u0131 kald\u0131\u011f\u0131ndan devaml\u0131l\u0131\u011f\u0131 ve yayg\u0131nl\u0131\u011f\u0131 hen\u00fcz sa\u011flanamam\u0131\u015ft\u0131r. Son y\u0131llarda acil proje say\u0131lar\u0131nda bir art\u0131\u015f olsa da kadavra kurslar\u0131, uygulamal\u0131 pratik e\u011fitimler, yurtd\u0131\u015f\u0131 akademik aktiviteler gibi di\u011fer bran\u015flarda s\u0131k g\u00f6r\u00fclen uygulamalarda bir art\u0131\u015f oldu\u011fu s\u00f6ylenemez.&nbsp;<\/p>\n\n\n\n<p class=\"rounded-3 px-3 py-2 has-pale-cyan-blue-background-color has-background\"><strong>\u201c\u0130la\u00e7 sekt\u00f6r\u00fc temsilcilerinin bu konuda beklentisi acil t\u0131p camias\u0131 i\u00e7inden daha fazla nitelikli proje fikri \u00e7\u0131kmas\u0131 y\u00f6n\u00fcndedir.<\/strong><strong>\u201d<\/strong><\/p>\n\n\n\n<p>Ancak sekt\u00f6r\u00fcn bir konuda bir handikap\u0131 oldu\u011fu da a\u00e7\u0131kt\u0131r ki yenilik\u00e7i projeler daha zor hayata ge\u00e7mektedir. Bu noktada yeni projelerde ba\u015far\u0131s\u0131zl\u0131k riskinin y\u00fcksek g\u00f6r\u00fclmesi al\u0131\u015f\u0131lagelmi\u015f projelerin daha fazla desteklenmesine yol a\u00e7maktad\u0131r denilebilir. Bu durum acil t\u0131p i\u00e7in geli\u015fmeyi engelleyen \u00f6nemli bir unsurdur. Zira tekd\u00fczelik artt\u0131k\u00e7a yeni yat\u0131r\u0131m alma oran\u0131n\u0131n d\u00fc\u015fece\u011fi \u00f6ng\u00f6r\u00fclebilir. Acil t\u0131bb\u0131 modern bir \u015firket gibi d\u00fc\u015f\u00fcn\u00fcrsek her y\u0131l ald\u0131\u011f\u0131 sekt\u00f6r deste\u011fi artmayan bir acil t\u0131p camias\u0131 zarar eden bir \u015firket gibi, al\u0131nan desteklerin azald\u0131\u011f\u0131 veya bazal d\u00fczeyde seyretti\u011fi bir statik bir yap\u0131ya d\u00f6n\u00fc\u015fecektir.&nbsp;<\/p>\n\n\n\n<p class=\"rounded-3 px-3 py-2 has-pale-cyan-blue-background-color has-background\"><strong>\u201cEkonomik zorluklar firmalar\u0131 acil servisler gibi zorlu ve y\u00fcksek riskli alanlardan uzakla\u015fmaya itmi\u015ftir\u201d<\/strong><\/p>\n\n\n\n<p>\u0130la\u00e7 sekt\u00f6r\u00fcn\u00fcn ekonomik olarak zorland\u0131\u011f\u0131n\u0131 da g\u00f6z ard\u0131 etmemek gerekir. Ancak bu zorlanman\u0131n a\u015f\u0131lmas\u0131 i\u00e7in se\u00e7ilen yollar sekt\u00f6r\u00fc asl\u0131nda \u00e7\u0131kmaza sokmaktad\u0131r. Ekonomik zorlanma firmalar\u0131 daha garanti g\u00f6r\u00fclen, y\u00f6netilmesi kolay se\u00e7eneklere y\u00f6nelmeye, acil t\u0131p gibi b\u00fcy\u00fck ama kendine has zorluklar\u0131 ve riskleri bar\u0131nd\u0131ran alanlardan uzakla\u015fmaya yol a\u00e7maktad\u0131r. Burada sekt\u00f6r\u00fcn as\u0131l yapmas\u0131 gereken ise destek politikalar\u0131n\u0131 g\u00f6zden ge\u00e7irmek olmal\u0131d\u0131r. Bug\u00fcn hi\u00e7bir firmada objektif ve etik a\u00e7\u0131dan tart\u0131\u015f\u0131lm\u0131\u015f destek kriterleri bulundu\u011fu s\u00f6ylenemez. Bu y\u00fczden verdikleri desteklerin da\u011f\u0131l\u0131m\u0131 da dengesiz veya hesaps\u0131z olabilmektedir.<\/p>\n\n\n\n<p>Acil klinikleri ile \u00f6zellikle ileti\u015fimin artmas\u0131n\u0131 sa\u011flayacak organizasyonlar yerine a\u011f\u0131rl\u0131kl\u0131 olarak b\u00f6lgesel d\u00fczeyde etkile\u015fimler a\u011f\u0131rl\u0131kl\u0131 olarak devam etmektedir. Tabi burada ileti\u015fimin zay\u0131f kalmas\u0131n\u0131n bir nedeni de firmalar\u0131n m\u00fcmessillerine verdi\u011fi hekim ziyaret hedeflerinde acil hekimlerinin oran\u0131n\u0131n d\u00fc\u015f\u00fck olmas\u0131d\u0131r. Ortalama 120 kadar hekim hedefi olan bir m\u00fcmessil i\u00e7in acil hekim hedefi bu de\u011ferin yakla\u015f\u0131k %5\u2019i olmaktad\u0131r. Tabiki bu de\u011fer ortalama bir acil klini\u011finin dahi hekim say\u0131s\u0131n\u0131 kar\u015f\u0131lamamaktad\u0131r. \u0130dealler hedefler ise bu rakam\u0131n m\u00fcmessiller i\u00e7in %20 civar\u0131nda olmas\u0131 kabul edilebilir bir de\u011ferdir. Bu sayede ila\u00e7 sekt\u00f6r\u00fc ve acil t\u0131bb\u0131n ileti\u015fiminin yeterli olaca\u011f\u0131 s\u00f6ylenebilir.&nbsp;<\/p>\n\n\n\n<p>Acil t\u0131p kanad\u0131nda da durum \u00e7ok farkl\u0131 g\u00f6r\u00fcnm\u00fcyor denilebilir. \u00d6zellikle kongre gibi b\u00fcy\u00fck organizasyonlarda s\u00fcrd\u00fcr\u00fclebilir mali politikalar hen\u00fcz hayata ge\u00e7iremedi\u011fi i\u00e7in y\u00fcksek maliyetler ila\u00e7 sekt\u00f6r\u00fcn\u00fcn destek kalemlerini yava\u015f yava\u015f eritmektedir. Bu durumun yans\u0131malar\u0131 kongrelerimizin b\u00fcy\u00fcme h\u0131z\u0131nda yava\u015flama hatta gerileme, ara sempozyumlar\u0131n daha zor organize edilmesi ve yurt d\u0131\u015f\u0131 organizasyonlar\u0131n azalmas\u0131 ile kendini g\u00f6stermektedir. Nitelikli yeni proje \u00fcretimi olduk\u00e7a yava\u015f ilerlemekte, ila\u00e7 firmalar\u0131 da aray\u0131\u015flar\u0131n\u0131 eski \u00e7al\u0131\u015fma alanlar\u0131na y\u00f6neltmektedir.&nbsp;<\/p>\n\n\n\n<p class=\"rounded-3 px-3 py-2 has-pale-cyan-blue-background-color has-background\"><strong>\u201cNitelikli proje temelinde ileti\u015fim kurulmal\u0131d\u0131r\u201d<\/strong><\/p>\n\n\n\n<p>\u0130la\u00e7 sekt\u00f6r\u00fcn\u00fcn yap\u0131lanmas\u0131na bak\u0131ld\u0131\u011f\u0131nda b\u00f6lgesel ve merkezi destek b\u00fct\u00e7elerine sahip oldu\u011fu g\u00f6r\u00fclebilir. B\u00f6lgesel b\u00fct\u00e7eler acil camias\u0131 i\u00e7in son derece fazla kullan\u0131l\u0131yor denebilir ancak merkezi b\u00fct\u00e7eler i\u00e7in ayn\u0131 \u015fey s\u00f6ylenemez. \u00c7\u00fcnk\u00fc merkezi planlamalar daha \u00e7ok b\u00fcy\u00fck ve ayr\u0131nt\u0131l\u0131 projelerde kullan\u0131l\u0131rken b\u00f6lgesel b\u00fct\u00e7eler kongre kat\u0131l\u0131m\u0131, temsil ve a\u011f\u0131rlama gibi harcamalarda kullan\u0131lmaktad\u0131r. \u0130la\u00e7 firmalar\u0131n\u0131n ko\u015fulsuz deste\u011fi ile bir proje y\u00fcr\u00fct\u00fcld\u00fc\u011f\u00fcnde ilk hareketlenme m\u00fcmessil kanad\u0131nda olmakta, sonras\u0131nda b\u00f6lge m\u00fcd\u00fcrleri ve \u00fcr\u00fcn m\u00fcd\u00fcrleri ziyaretleri artmaktad\u0131r. Projeye dahil olan t\u00fcm merkezler ila\u00e7 tan\u0131t\u0131lan bir alan\u0131n \u00f6tesine ge\u00e7mekte ve bir i\u015f ortakl\u0131\u011f\u0131 modeli olu\u015fmaktad\u0131r. Asl\u0131nda kurulmas\u0131 gereken en sa\u011fl\u0131kl\u0131 ili\u015fki bu gibi g\u00f6r\u00fcnmektedir. \u00c7\u00fcnk\u00fc \u00fcretilen projelerin neredeyse tamam\u0131 akademik ve acil t\u0131bba fayda sa\u011flayacak projeler olmaktad\u0131r. Do\u011fru zeminde kurulan ili\u015fkiler de daha fazla ileti\u015fim sa\u011flamakta ila\u00e7 firmalar\u0131n\u0131n klinikleri daha iyi tan\u0131mas\u0131yla \u00fcst d\u00fczey desteklerin \u00f6n\u00fc a\u00e7\u0131lmaktad\u0131r. Dolay\u0131s\u0131yla her klinik kendi \u00e7at\u0131 projesini belirleyerek sekt\u00f6rle ileti\u015fim sa\u011flad\u0131\u011f\u0131 noktalarda bu projelerin birlikte yap\u0131lmas\u0131 konusunda \u00e7aba g\u00f6stermelidir. B\u00f6ylece m\u00fcmessil asl\u0131nda bir proje temsilcisi, firma da bir i\u015f orta\u011f\u0131 olarak \u00e7al\u0131\u015fmaya ba\u015flayacakt\u0131r.&nbsp;<\/p>\n\n\n\n<p>Pandemi ko\u015fullar\u0131n\u0131n yaratt\u0131\u011f\u0131 ekonomik dengelerde ila\u00e7 sekt\u00f6r\u00fc ve acil klinikleri aras\u0131nda \u00e7e\u015fitli ek zorluklara yol a\u00e7m\u0131\u015ft\u0131r. Bu zorluklardan en \u00f6nemlisi acil serviste yaz\u0131lan bir\u00e7ok ilac\u0131n (\u00f6rne\u011fin topikal ila\u00e7lar) sosyal g\u00fcvenlik kurumu \u00f6deme listesinden \u00e7\u0131kar\u0131lmas\u0131d\u0131r. Bir di\u011fer \u00f6nemli konu da firmalar\u0131n maddi dar bo\u011faza girmesiyle karl\u0131l\u0131k oranlar\u0131n\u0131 art\u0131rma \u00e7al\u0131\u015fmalar\u0131nda g\u00f6ze \u00e7arpan uygulamalard\u0131r.&nbsp;<\/p>\n\n\n\n<p>Firmalar daha ucuz ve \u00f6denmeme ihtimali olan bir\u00e7ok ilac\u0131 bir m\u00fcmessil grubuna \u00e7al\u0131\u015ft\u0131rmaktansa, bu ila\u00e7lar\u0131 m\u00fcmessilden al\u0131p di\u011fer bran\u015flar\u0131n \u00f6demesi daha y\u00fcksek olan ila\u00e7lar\u0131n\u0131 vererek tek m\u00fcmessilden elde edilen karl\u0131l\u0131\u011f\u0131 art\u0131rmaya \u00e7al\u0131\u015fmaktad\u0131r. Bu durumda m\u00fcmessilin elinde acil servise \u00e7al\u0131\u015faca\u011f\u0131 \u00e7ok az say\u0131da ila\u00e7 kalmakta zaman\u0131n\u0131 ve akademik destekleri di\u011fer bran\u015flara kayd\u0131r\u0131lmaktad\u0131r. Bu uygulamalar\u0131n k\u0131sa ve uzun vadede acil t\u0131p kliniklerinin sekt\u00f6r ile temas\u0131n\u0131 azaltaca\u011f\u0131 a\u00e7\u0131kt\u0131r.\u00a0<\/p>\n\n\n\n<p><strong><em>Nihayetinde acil t\u0131p ve ila\u00e7 sekt\u00f6r\u00fc ileti\u015fiminde yap\u0131lmas\u0131 gerekenlerde ilk s\u0131rada acil t\u0131p camias\u0131ndan k\u00f6ken alan kaliteli proje fikirlerini art\u0131rmak gelmelidir.<\/em><\/strong> Bu projelerin uygulanmas\u0131nda da mutlaka taraflar aktif olarak g\u00f6rev ve sorumluluk alarak tek tarafl\u0131 planlanm\u0131\u015f organizasyonlar azalt\u0131lmal\u0131d\u0131r.
